Your rights include the following:
Legal Rights
You maintain the same legal rights guaranteed by State and
Federal constitutions, unless any are limited by a court. You will not be discriminated against because
of race, color, national origin, gender, sexual orientation, religion, age,
disability or degree of handicap, or ability to pay.
Personal Rights & Responsibilities
You have the right to be treated with dignity and respect,
to be free from abuse, coercion and exploitation at all times. This means that you have the right:
- To be
treated with respect for your
privacy and the confidentiality of information about your treatment.
ONH values a collaborative, team-based approach to providing
services. When clinically
appropriate, information about your needs and goals may be confidentially
shared with other ONH providers to guide development and provision of the
most suitable service plan for you. Information will not be disclosed to any outside
party without your permission, unless it is required by court order or
law.
Please Note: The
State of New Hampshire
mandates the involuntary release of confidential information under specific
circumstances:
- ONH is
required to report any suspicion of abuse or neglect of children to the
appropriate state agencies.
- Any
threats of violence or destruction towards self, others, or property may
be reported to the intended victim or the police having local jurisdiction.
You have the right to review, with a written request, your
record with a member of your treatment team who can answer any questions you
might have, and have a copy made available to you at your cost.
2. To participate in
development of an individualized treatment plan. Based on initial and ongoing evaluations of
your needs and goals, you will help develop, review and revise a plan detailing
services and treatments recommended to assist you in meeting your goals. You have the responsibility to participate as
actively and honestly as you can, and keep your treatment providers informed of
any changes in your needs or goals. You
also have a responsibility to ask questions about your treatment so that you
fully understand it.
3. To be treated in a
safe and secure environment. You
will receive services in the most therapeutic setting available, one which
encourages health, fosters independence and choice, and promotes progress
toward your goals. You have the
responsibility to assist in creating a safe environment for yourself and
others. If necessary, your freedom of
choice may be limited, but only as needed to provide a secure environment. Threats of violence or destruction, and
weapons are not permitted in or on ONH property or at any ONH sponsored
activity.
4. To complain or
bring a grievance if you think that your rights have been violated, and have this investigated. The complaint
may be written or verbal, and forwarded to:
